Instructions for Becoming a New Provider

To apply to be a provider of DUI Intervention Program services either as a clinical evaluator or as a treatment provider you will need to:
  • 1. Complete the Online Application Form in Full and Submit. Click here to fill out the New Provider web form

    By filling this out you will be assured of getting on the the Registry. The Registry provides a way for potential clients to find out about your services and qualifications.

  • 2. Then Print the PDF Application Form, Complete and Sign. Click here to get the paper application form. Then return this form with all the requested documents, photograph and check amount to Scott Dunbar, DUI Intervention Program, Office of Addictive Diseases, MHDDAD, 2 Peachtree Street, NW, 22-285. Atlanta, Georgia, 30303.
